Patient Portal

You can now easily access upcoming appointment information, billing statements, and refill requests through our online patient portal by using the link below. If you have difficulty logging into the portal, please contact the front desk by emailing [email protected]

If you are an established patient looking to request additional services, please log into the portal and submit a message to Intakes. Please allow 3 business days for our Intake Coordinator to reply to your message via the secure patient portal to inform you of current clinician availability and discuss whether or not you'd like to remain on a waitlist if applicable.
